The Heckler and Koch Mark 23 is a rare handgun that is chambered in .45 ACP Ammo, the highest damaging caliber for handguns. It uses a 12-round magazine also used by the USP45 and G37. However, It only accepts the .45 ACP silencer, unable to house sights or underbarrel attachments.

- It does not have illuminated iron sights. This makes it hard to find your target at night. Guns such as the M9 and USP45 have these circular lights on them in-game.
